Join the Yukon Home Care Program as a Community Liaison Coordinator in Whitehorse. Utilize your skills in physiotherapy or occupational therapy to enhance home care services in remote communities.
This is a full-time role offering an exciting opportunity to supervise home support workers and coordinate care for Yukoners outside of Whitehorse. You will conduct assessments, manage cases, and lead discharges from hospitals. Ideal candidates are flexible communicators with superior problem-solving and leadership abilities, contributing to a culturally safe work environment.
Key Responsibilities:• Supervise home support workers in rural communities • Coordinate hospital discharges and long-term care assessments • Perform case management for various community needs • Collaborate with interdisciplinary teams to improve services • Engage actively with community partners to enhance care
Requirements:• Master’s degree in Physiotherapy or Occupational Therapy • Current registration with a Canadian regulatory body • Experience in program planning and staff supervision • Excellent interpersonal and organizational skills • Ability to travel regularly to remote communities
