Case Management Social Worker - Autism Focus, Whitby

Nova’s Ark seeks a Registered Social Worker to provide case management, care coordination, and family support for children and youth in the Ontario Autism Program. Make a meaningful impact on families through effective collaboration and resource guidance.

In this Registered Social Worker role, you will guide families through service navigation while providing essential assessments and emotional support. Work alongside an interdisciplinary team, participate in case conferences, and mentor interns in a caring environment. Your professional expertise will help families build the capacity they need to succeed.

Key Responsibilities:• Manage cases of participants in the Ontario Autism Program • Conduct assessments of psychosocial and family needs • Facilitate advocacy and resource access for families • Offer emotional support and short-term counseling • Collaborate with caregivers and community professionals

Requirements:• Bachelor’s or Master’s degree in social work • Valid RSW registration with OCSWSSW • Experience with autism or developmental services • Strong organizational and documentation abilities • Willingness to acquire CPR and CPI certifications
