Providence Health Care is now hiring a Patient Care Manager to support the opening of the next phase in Road to Recovery (R2R) continuum of person-centered and compassion-driven services: a ~50 bed off-site treatment center. In this role, you’ll play a pivotal role in shaping how this program comes to life—leading planning, designing care delivery, and ultimately overseeing a high-performing interdisciplinary team supporting clients through treatment and recovery. The R2R initiative strives to better address treatment of substance use issues by providing the right support to each client at the right time. People who use substances deserve the same access to evidence-based care as anyone else, free from stigma. As with all of our services at Providence Health Care, R2R delivers care in a way that strives to be person-centered, trauma-informed and culturally safe. Launched in 2023, the R2R supports clients at every phase of their wellness and recovery journey by coordinating seamless care, even if there are setbacks or relapses along the way.The R2R continuum of services in operation today includes 25 detox beds located at St. Paul’s Hospital, 20 transitional care beds for clients awaiting space in a treatment program or long-term housing, and the Aftercare Team, which launched in Fall 2024, providing outreach aftercare in the community.Your duties will include:

Lead the planning and launch of a new treatment program: Play a key role in shaping a new substance use treatment facility—working with clinical and project leaders to design care models, workflows, staffing structures, and operational processes that support protected, effective, and recovery-oriented care.Oversee day-to-day operations and team leadership (post-launch): Provide operational leadership for an interdisciplinary team, ensuring high-quality, patient-centered care delivery while managing staffing, budgets, and overall program performance. Drive quality improvement and service excellence: Use data, quality improvement, and risk management strategies to continuously evaluate and enhance care delivery—ensuring services are efficient, evidence-informed, and responsive to the needs of clients and families.Build strong partnerships across the care continuum: Collaborate with internal teams and community partners to support seamless transitions in care, strengthen connections to treatment and recovery services, and ensure clients are supported throughout their journey. What You Bring

Masters degree in a related Health Profession plus a minimum of five (5) years’ experience at the middle management level in a medium to large health care organization or an equivalent combination of education and experience. Community substance use services experience preferred, especially treatment/recovery services.Clinical service design and planning experience an asset. Leadership: Defines a vision and facilitates, guides, and mentors individuals and groups towards the vision, while maintaining group cohesiveness, motivation, commitment and effectiveness. Communication: Communicates effectively both verbally and in writing so that messages are understood; that is, understanding the essence and subtleties of the communication and the intended audience.Resources Management: Manages human, capital, financial and information resources so that organizational objectives are achieved. Conceptual Skills: Identifies and analyses situations and problems such that viable solutions are found. Approaches tasks and problems such that total systems and strategies are taken into account.Public/Community Focus: Responsive to the needs of health care, public, and community stakeholders and customers. Seeks partnership opportunities and actively promotes positive relations. Results Management: Plans and establishes courses of action for self and others that are results oriented. Systems Thinking: Aware of the interdependence of organizational systems and stakeholders and considers the whole in the formulation of solutions.Team Focus: Strives to understand perspectives of team members, synthesizes with own opinion, presents convincing point of view, and resolves conflicts. Time Management: Manages own work activities and delegates tasks to achieve maximum efficiency. Mastery of Change: Accepting of new initiatives and champions the implementation process amongst team members.Quality Improvement: Continuously identifies and implements improvements in work systems to ensure that the highest possible level of quality service is achieved. Competitive Pay: $118,923 - $170,951 CAD per year, depending on experience.
