Under the System of Care’s Program Manager, this role requires collecting and organizing client data for the Coordinated Access System. You will serve as the primary contact for those entering the system, ensuring compliance with appropriate policies. Engage in intensive support for individuals and families experiencinghomelessness while promoting their dignity and personal needs.

Key Responsibilities:• Collect and manage client and system-level data • Process referrals from community partners effectively • Conduct intakes with clients in person or over the phone • Follow up with service providers regularly • Act as a HIFIS super user and trainer

Requirements:• Diploma or degree in Social Work or related fields • Strong written and verbal communication skills • Familiarity with homelessness issues and client interviews • Experience with spreadsheet software and databases • Valid driver’s license and ability to travel regularly
