Registered Practical Nurse, Patient Navigator Misiway is an Indigenous-led Primary Health Care Organization in Timmins, Ontario. Misiway, meaning“everyone” in Cree, reflects our commitment to inclusive, community-centred care. We support the whole person by bringing together people, culture, and services to ensure care for everyone.
Status: Full-Time, Permanent (35 hours per week)
Salary Range: $49,400 - $59,300
About the Role The RPN/Patient Navigator is a key member of Misiway’s Integrated Care Team. This role blends clinical nursing practice with system navigation to support clients living with complex chronic conditions, serious illness, and life-limiting diagnoses.
Working within a wholistic model of care that honours the physical, emotional, mental, and spiritual well-being of clients, the Navigator/RPN applies nursing knowledge to care across the continuum. This role supports clinical monitoring, client education, and care coordination, while helping clients and families navigate health systems and access culturally appropriate supports.
Grounded in cultural safety, trauma-informed practice, and Two-Eyed Seeing, this position brings together Indigenous and Western approaches to care with equal respect.
What You’ll Do Clinical Care&Navigation
Provide nursing care within RPN scope of practice, including health assessments, monitoring of chronic conditions, medication support, and basic clinical interventions (e.g., wound care, vital signs, symptom monitoring)
Support clients in understanding their diagnoses, treatment plans, and care plans
Navigate clients through health care systems, including primary care, specialists, home care, and community services
Coordinate care across providers to ensure a seamless, client-centred experience
Support safe and timely transitions in care, including hospital to home
Care Coordination&Client Support
Reinforce care plans developed by primary care providers and support follow-through on recommended treatments and referrals
Monitor and report changes in client condition to the care team, supporting timely follow-up
Facilitate communication between clients, families, and providers to ensure clarity and continuity of care
Provide practical support and health education to clients and families to build confidence in managing health conditions
Barrier Reduction&Wholistic Support
Identify and respond to barriers to care, including transportation, housing, food security, and access to services
Connect clients to community-based, cultural, and wellness supports that address well-being beyond the clinical encounter
Facilitate access to Traditional Healers, Elders, cultural programming, and social services as desired and appropriate
Maintain accurate and timely documentation in Misiway’s ElectronicMedical Record (EMR)
Participate in interdisciplinary team meetings, case conferences, and care planning
Uphold client privacy and confidentiality in accordance with PHIPA and organizational policies
What You Bring Required
Current registration in good standing with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse (RPN)
Diploma in Practical Nursing
Minimum two (2) years of experience in a clinical or community health setting
Experience supporting clients with complex chronic disease or ongoing health needs
Strong clinical judgment, communication, and organizational skills
Ability to work both independently and as part of an interdisciplinary team
Proficiency with computers; willingness to learn Misiway’s EMR system
Valid Class G Ontario Driver’s Lisence and access to reliable transportation
Assets
Experience working with Indigenous clients and communities, with knowledge of Indigenous cultures, healing practices, and the impacts of colonialism and intergenerational trauma
Experience in chronic disease management, wound care, or community/home care nursing
Ability to speak Cree or Ojibway
Lived experience navigating health or social systems as a patient, caregiver, or community member
Standard First Aid and CPR certification
