Qualifications

Registered and in good standing with the College of Dietitians of Ontario

Must provide proof of liability insurance

Must provide proof of TB testing

Must provide proof of a vulnerable sector check

Ability to work both independently and within a multidisciplinary team

Strong written and verbal communication skills

Excellent organizational and time‑management abilities

Demonstrated problem‑solving skills

Proficiency with computers, including experience using electronic medical records (EMR)

Travel may be required for certain positions

Responsibilities

Identify and monitor the level of nutrition and hydration risk for all residents

Complete scheduled RAI assessments on admission, quarterly and for significant status changes

Prioritize and respond to dietary referrals in a timely manner

Develop, evaluate and modify individualized nutrition care plans

Liaise with residents, families/SDMs, and the team to implement effective dietary interventions

Conduct swallowing assessments as required

Provide nutrition counselling and education to residents and their families

Collaborate with the Nutrition Manager to enhance the dining experience

Participate in care conferences and committee meetings as per organizational needs

Review and approve seasonal menus and create individualized menus where applicable

Provide training and support to the dietary and nursing team

Contribute to continuous quality improvement (CQI) initiatives

Ensure all work complies with Ontario Regulation 246/22, Section 80, under the Fixing Long‑Term Care Act, 2021

Perform additional duties as required
