Join the mission to enhance seniors' wellbeing as a Community Connector. Connect older adults 60+ to community resources that promote health, independence, and social engagement in Richmond.
As a vital resource, you will engage with seniors and empower them to access services tailored to their needs. Responsibilities include managing caseloads, assessing risks, and developing supportive routines. Collaboration with community networks and healthcare professionals is key to providing seniors with the support they need to thrive.
Key Responsibilities:• Manage a dynamic caseload of seniors needing assistance • Determine individuals in need of community program access • Conduct thorough client intake assessments • Establish routines to minimize risk of frailty • Strengthen community relationships for senior service access
Requirements:• Bachelor’s degree in social services or equivalent • Experience with seniors and diverse populations • At least two years of related experience • Strong communication and conflict resolution skills • Valid driver's license and reliable transportation
