Bayshore HealthCare is seeking an experienced professional for the Integrated Care Solutions team. In this leadership role, you will act as the primary clinical resource for coordination teams, guiding best practices and addressing urgent client needs. Your expertise will enhance care delivery while ensuring compliance with standards and fostering quality improvements.
Key Responsibilities:• Act as the primary clinical resource for coordination teams • Manage daily escalations from clients and partners • Review client files for accuracy and compliance • Provide real-time coaching to coordination staff • Support follow-up on hospital discharge and referrals
Requirements:• 5+ years in healthcare, preferably in home care settings • Registered Practical Nurse with CPR certification • Knowledge of Ontario’s health system and community nursing • Proven leadership and mentorship skills • Valid driver’s license and reliable vehicle
