Key Responsibilities:• Monitor nutrition and hydration risk levels • Complete RAI assessments on scheduled timelines • Develop and modify individual nutrition plans • Collaborate with families and the dietary team • Conduct nutrition counseling for residents

Requirements:• Good standing with the College of Dietitians of Ontario • Proof of liability insurance required • Strong time-management abilities • Excellent problem-solving skills • Ability to use electronic medical records proficiently
