Social Worker
(Fraser Health Authority)– Hope, BC The salary range for this position is CAD $42.27 - $52.81 / hour. Collaborates with the Manager, Health Services to develop, maintain and evaluate social work clinical practice standards, policies, and procedures. Assumes the clinical caseload for Fraser Hope Lodge and Fraser Canyon Hospital and manages referrals from staff, community-based agencies, self and/or family referrals. Plans, implements and evaluates treatment of clients in accordance with Fraser Health standards and the Social Work Code of Ethics.Models and facilitates a client-focused, goal-directed, interdisciplinary professional practice model. Responds to referrals from emergency and reviews admissions to acute, sub-acute, and hospice for high-risk indicators. Conducts comprehensive social work and psycho-social assessments and liaises with community agencies to ensure continuity of care for patients and residents.Follows up and provides ongoing monitoring of complex psychosocial situations and evaluates patient response to interventions across the continuum of care. Maintains patient records using approved formats, documenting assessments, progress notes and reports according to standards and procedures. Acts as a resource for patients and families by providing information on community resources, facilitating referrals, and identifying services to support health needs.Collaborates with the interdisciplinary team to identify service gaps and contribute to quality improvement initiatives as directed. Participates in interdisciplinary and multi-agency meetings and discharge planning for complex plans; liaises with Home Health regarding discharge planning and Home Supports as needed.Performs other related duties as assigned. Qualifications
Education and Experience Master's Degree in Social Work from an accredited school of Social Work. Three (3) years of recent clinical experience or an equivalent combination of education and experience. Current full registration with the BC College of Social Workers. Valid Class 5 driver's license with access to a personal vehicle for business purposes.Skills and Abilities Knowledge of counseling skills and clinical practice models for individuals, families, and groups. Understanding of the scope of practice of other health disciplines. Ability to conduct social and psycho-social assessments and crisis intervention. Ability to make effective referrals and work collaboratively with community resources.Ability to educate and provide consultation to patients, families and health professionals; strong written and verbal communication skills. Ability to organize and prioritize workload in a complex environment; collaborative, multi-cultural team orientation. Ability to handle conflict and maintain professional boundaries; physical ability to perform duties.Applicants with a Bachelor of Social Work (BSW) and a demonstrated equivalent combination of education and experience may be considered.
